Maternal stress can lead to a strong allergic response in their infants, even if they had little
allergen exposure, according to research presented at the American Thoracic Society's International Conference in
Toronto, Canada. In a related study presented at the same meeting, firstborn children carrying a genetic variant
are more likely to have higher allergy responses. In addition, a study presented on May 20 identifies a possible
link between cesarean delivery and the risk of allergy and asthma in childhood, and a study presented on May 21
shows that children whose mothers were exposed to farms are protected from allergies.

Under Ohio’s Healthcare Simplification Act, which goes into effect on June 25, health
care plans are required to be clearer and more open about contract terms with physicians than they have been in the
past, disclosing what insurers will pay for services, and notifying doctors of significant changes to the contract.
The law also places limits on so-called silent PPOs: the health plan practice of paying for access to other
companies’ network physicians, then reimbursing physicians at the lowest available
rate.

The ethical ramifications of physicians serving as commercial endorsers was spotlighted recently when
Pfizer was pressured into terminating a massive advertising campaign for Lipitor (atorvastatin) featuring Robert
Jarvik, MD, in the wake of a US House Energy and Commerce Committee probe. Pfizer ended the campaign after Rep.
John Dingell (D, Mich.), chairman of the committee, charged that the ads misled the public because they created the
impression that Dr. Jarvik, one of the developers of the Jarvik-7 artificial heart, is a practicing physician. In
fact, Dr. Jarvik is not licensed to practice medicine. The committee disclosed that Dr. Jarvik had a 2-year
contract to receive $1.35 million for his participation in the campaign, according to American Medical News
(3/31/2008). Dr. Jarvik defended his role as a physician spokesman, saying in a statement that his
“credibility as a heart expert is fully justified and is fairly represented” in the
ads.

Yes, according to the results of a UK study involving over 27,000 primiparous women. The authors of
the study found that women with a cervix measuring 40 to 67 mm at a median of 23 weeks’ gestation
were almost twice as likely to require a cesarean delivery at term because of poor progress in labor as women with
a mid-pregnancy cervical length measurement of 16 to 30 mm (odds ratio [OR] 1.81, 95% CI, 1.66–1.97).
Adjusting for maternal age, bodymass index, smoking status, race or ethnic group, gestational age at birth,
spontaneous or induced labor, birthweight percentile, and hospital of delivery still left these women more than
one-and-a-half times more likely to require the surgery as the women with shorter cervices (OR 1.68, 95% CI,
1.53–1.84; P<0.001).

Physicians’ groups and insurers recently agreed to develop a national set of standards
for measuring doctors’ performance. The agreement represents a significant step in resolving a
longstanding dispute about current rating practices. Physicians charge that health plans focus too much on cost
rather than quality of care and that the rankings are often inaccurate, according to The New York Times (4/2/08).
Leading physician groups, including the American Medical Association, American College of Surgeons, and American
College of Physicians, signed on to the agreement, along with major health insurers, such as Aetna and United
Healthcare. The broad coalition endorsing the new plan also includes consumer, labor, and employer

Physicians are willing to share knowledge about harmful errors and near misses with their institutions
but find current systems to report and disseminate this information inadequate. So they rely instead on informal
discussions with colleagues, according to a survey of more than 1,000 physicians, as reported in the Medical
Liability Monitor (3/2006). As a consequence, much important information remains invisible to institutions and the
health care system, investigators concluded, noting the importance of gathering comprehensive information about all
errors and near misses to understand and prevent medical errors. Results of the survey, which focused on attitudes
about communicating errors as well as experience with errors, were published in the February/March issue of Health
Affairs.

In 1999, a 38-year-old Illinois woman had an uneventful prenatal course until almost term when she
presented with back and abdominal pain and persistent vaginal bleeding. She was admitted to the hospital for
evaluation and after about 1 hour of monitoring, it was determined she was not in labor and the FHR was reassuring.
The woman was discharged home despite continued bleeding and some abdominal pain. When the patient returned to the
doctor's office that afternoon with complaints of increased bleeding and pain, she was advised to go to the
hospital. While in route, she had a large amount of bleeding just before arriving at the hospital. A fetal demise
was diagnosed from placental abruption before an emergency C/S could be performed. The patient required
transfusions for DIC.

A 39-year-old Pennsylvania woman was admitted at term for delivery in 1997. She was managed by a
certified nurse midwife and obstetrician and after 8 hours of labor the infant was delivered by cesarean section.
He had no heart rate, and was pale and limp. Despite resuscitation, when a neonatologist arrived 11 minutes later,
the infant was subsequently diagnosed with cerebral palsy with global developmental delay and
quadriplegia.
